29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed: On or about August 3, 2017, employees were exposed to struck-by and amputation hazards when there had been no inspection of the machine specific lockout/tagout procedures since 2015 for equipment such as, but not limited to, Norfield magnum, Kval 95 hinge prep machine, Kval 920 strike prep machine, Norfield 5000, Norfield 4000, Norfield Eagle, Norfield 1020. No abatement certification or documentation is required for this item.
