29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was possible: The employer failed to prevent employee exposure to amputation and struck-by hazards. On or about 08/02/2017, at 5580 Vinmil Road, House Springs, MO, Aim to Amaze Outdoor Services Inc. employee was exposed to hazards such as those created by rotating parts and flying wooden chips. The Bandit 200 wood chipper's rotating wheel blade assembly guard/cover was not affixed to the machine exposing the employee to the rotating assembly and possible flying wood chips.
