29 CFR 1910.178(m)(2): No person shall be allowed to stand or pass under the elevated portion of any truck, whether loaded or empty. 918 N. Porter St. in Streator, IL 61364, warehouse: on or about August 2, 2017, employees assigned to unload plastic pellets from bulk bags (super sacks) handled by powered industrial trucks (forklifts) were required to get under elevated portions of loaded trucks to untie the bottom discharge chute on super sack's for transferring product, and to retie it during active product transfer. Employees were exposed to crushing hazards from failing/lowering forklift's components (forks, lifting carriage, mast) and/or falling load (super sack). Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
