29 CFR 1910.177(g)(6): A procedure was not developed for ensuring single piece rim wheels were contained within a restraining device, positioned behind a barrier or bolted on the vehicle with the lug nuts fully tightened during tire inflation: (a) 820 Petersburg Road, Carlisle, PA - The employer did not establish a procedure that required the single piece rim wheel to be contained in a restraining device, positioned behind a barrier or the lug nuts fully tightened. On July 10,2017 employees inflated a tire with the lug nuts installed with only a few threads engaged onto the studs. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
