29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) 195 Athens Highway, Loganville, GA: On or about July 10, 2017, and at times prior, the employer exposed employees to engulfment/cave-in hazards, in that entry and egress within a 7 feet trench extended a foot above the landing while employees were installing utility lines. b) 195 Athens Highway, Loganville, GA: On or about July 10, 2017, and at times prior, the employer exposed employees to engulfment/cave-in hazards, in that entry and egress within a 9 feet trench extended a foot above the landing while employees were installing piping.
