29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval and capacity, operation and maintenance instruction plates, tags or decals were not changed accordingly: DPU6: On, or about, March 10, 2017, a pole style attachment was used on Hyster forktruck serial number A955N02885N to lift rolls of conveyor material. The estimated reduced capacity of the forktruck when using the pole attachment was calculated to be 1,588 pounds. The larger roll of conveyor material weighed 1,205 pounds. The forktruck was not marked to show the reduced capacity of the forktruck when using the pole attachment. Abatement Note: All attachments used with the forktruck must be included on the identification/capacity plate on the truck.
