29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Employees are exposed to caught-in and amputation hazards when working on loaders in building . The employer has not developed, documented or utilized any procedures for the control of potentially hazardous energy. Employees engaged in maintenance activities have no procedures to follow when working on ammunition loading machines. The employee sometimes lockout a disconnect, usually just unplug, while sometimes when making small adjustments leave the machine as is and make the adjustment.
