29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points and rotating parts: (a) On or about May 27, 2017, the Heidelberg Harris Print Stream Press was not equipped with a guard on roller unit #2, which resulted in a crushing injury to an employee's left thumb and pointer finger when the employee reached into the roller with a rag to wipe it clean and made contact with the ingoing nip point of the moving press roller. No abatement certification or documentation is required for this item.
