29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. Location: a) Supported scaffold on the roof at northwest side of the job site On or about 05/19/17, a supported scaffold was not erected under the supervision and direction of a competent person qualified in scaffold erection. Employees were exposed to fall hazards due to deficiencies with the scaffold such as but not limited to: missing base plates, not leveled, mounted on unstable objects, no safe means of access to the scaffold platform, not protected from falls by use of personal fall arrest systems or guardrail systems.
