29 CFR 1910.147(c)(5)(ii)(D): Lockout devices and tagout devices did not indicate the identity of the employee applying the devices. On or about 1 May 2017 and at times prior thereto, lockout devices used to isolate energy sources did not indicate the identity of the employee applying the device, exposing the employees to that hazards of being struck by or caught in moving machine parts. A) The Large Gluer has 3 keyed locks, all with blue wrist straps for the keys and no means to indicate the identity of the employee applying the lock. B) The Jumbo Slotter/Printer has 4 keys with 3 different colored wrist straps, one with no attachment, but no means to indicate the identity of the employee applying the key.
