29 CFR 1910.179(j)(2)(iv): Monthly inspections of hoist chains, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected, were not performed: The employer is failing to protect employees from caught-under and struck-by hazards associated with using chains to lift and suspend loads. The most recent occurrence was evident during the inspection conducted on May 4, 2017 at the facility located at 13433 Centech Rd, Omaha, NE 68138. The employer has employees lifting loads up to 10 tons using chains which have not had a monthly inspection with a certification record.
