29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: Through the facility - On or about April 21, 2017, employees using the Toyota sit-down type LP-gas forklift, model 7FGCU25, SN #73736 with an personnel platform to lift employees to perform maintenance tasks; exposing them to falls from distances up to 20' feet to the concrete below. The platform was not approved by the manufacturer. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
