29 CFR 1910.147(f)(3)(ii)(D): A personal lockout or tagout device(s) was not affixed to the group lockout device, group lockbox or comparable mechanism when he or she began work (and to be removed when work stops) on the machine or equipment serviced and/ or maintained. a) STTA In-Line Machining Center #20, Production Floor: On or about March 28, 2017 and at times prior, employees were exposed to caught-in, struck-by, and crushed-by hazards, when performing servicing, setup, and maintenance activities on equipment including, but not limited to, STTA In-Line Machining Center #20 which had hydraulic, pneumatic and electrical energy sources, in that each employee did not apply a lockout device on the energy control lockout point.
