29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: a) Piggy back forklift: On or about March 23, 2017 and at times prior thereto, the employer did not ensure refresher training and evaluation of the operator's performance was conducted again after the initial training about 5-years prior to this inspection which exposed employees to forklift related hazards such as but not limited to rollovers while using on uneven or included surfaces.
