29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. a. On or about April 5, 2017, located throughout Plant #8, employees operated forklifts with spinner knobs attached to the forklift's steering wheel without the manufacturer's prior written approval. The spinner knobs affected the safe operation of the forklifts, thereby exposing the employees to the hazards associated with forklift operations.
