29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about February 24, 2017, at the above addressed worksite, employees were performing set-up, cleaning and maintenance on commercial bakery machines, such as but not limited to; a bagel machine, a loaf cake machine, a round cake machine, two horizontal bagel dough mixers, five vertical cake dough mixers and a dough sifter, for which the employer had not developed and implemented a Hazardous Energy Control program/Lock-out Tag-out program; therefore exposing the operators, the mechanic and cleaning/sanitation employees to amputation hazards. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
