29 CFR 1910.146(d)(3): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations: The employer is failing to protect employees from the hazards associated with safe means of entry, procedures, and practices for employees to enter permit required confined spaces. This was most recently documented on February 1, 2017 at 9100 West Liberty Drive, Pleasant Valley, Missouri 64068. The employer is not protecting employees during the tank cleaning process in that the procedures developed by the employer do not identify conditions of breaking the plane with any part of the body, to include the face, as making entry into the permit-required confined space.
