29 CFR 1910.212(a)(3)(ii): The employer did not ensure point of operation guards were designed and constructed to prevent the operator from having any body part in the danger zone during the operating cycle: Line 3 Rotary Mold Machine- On or about January 31, 2017 employees are exposed to caught-in and crushed hazards while pushing dough through guards with rectangular openings which measure approximately 1 5/8" by 3 1/4". The guard over the two in-going rolls was not adequate to prevent the hands of operators from entering the point of operation. Abatement documentation per 1903.19(d) is required for this item.
