29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by a competent person prior to the start of work and as needed throughout the shift: (a) Wyvern Site, Track Location OOC 418.1 - On or about January 24, 2017, employees were working in an unprotected trench, ranging from 6-9 feet, with walls that had been cut almost vertically. The inspection performed by CSX did not properly identify situations that could result in cave-ins.
