29 CFR 1910.119(f)(1): The employer did not develop and implement written operating procedures that provided clear instructions for safely conducting activities involved in each covered process consistent with the safety information and which address the elements listed in 29 CFR 1910.119(f)(1)(i) through (f)(1)(v): a.) KC Midstream Solutions, LLC, 101 Queen Creek Rd, Endeavor, PA - On or about January 11, 2017, and at times prior thereto: employees were potentially exposed to fire and/or explosion hazards in that, the employer had not developed and implemented operating procedures for process equipment including, but not limited to, the fired heater, pressure relief valves, the 30,000 gallon hydrocarbon storage tank, and associated piping.
