29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): Location: FSS Building The Toyota #16 forklift was in service and operating with a defective, non-functioning horn. Employees were exposed to struck-by hazards from the warning notification signal inoperable on the powered industrial vehicle. The U.S. Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1910.178(p)(1)], which was contained in OSHA inspection number 1146372, citation number 1, item number 2 and was affirmed as a final order on November 1, 2016, with respect to a workplace located at 5555 Bandini Boulevard, Bell, California 90201. --IN THE ALTERNATIVE-- 29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service: Location: FSS Building The employer did not ensure that employees performed a pre-shift inspection before use on powered industrial vehicles, such as but not limited to the Toyota #16 forklift. Employees were exposed to struck-by hazards from the warning notification system inoperable on the powered industrial vehicle. The U.S. Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1910.178(q)(7)], which was contained in OSHA inspection number 1153566, citation number 1, item number 2 and was affirmed as a final order on November 16, 2016, with respect to a workplace located at 15421 Gale Avenue, City of Industry, California 91715.
