29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation. On or about October 31, 2016 and at times prior thereto, at the worksite located at 801 N. Jefferson Street, Springdale, Arkansas, a moulding machine operator was not trained in lockout/tagout procedures prior to operating the machine. This condition exposed the employee to amputation hazards from rotating parts as employees passed by guards to make machine adjustments.
