29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): Tire Shredder Machine Area: Employer did not include in their PPE assessment overhead hazards present where employees work on or near the tire shredder machine area such as operating the conveyors, clearing jams in the chute and conveyors, feeding tires onto the conveyors and shoveling debris off the floor and runways and were exposed to falling objects such as tire parts and tools used to clear jams and maintenance.
