29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: The employer is failing to protect employees from potential eye injury hazards caused by struck-by or splash hazards. This was most recently documented on October 17, 2016 at a worksite located at 7723 Heritage Highway, Jefferson City, Missouri where employees were working without the use of eye protection while pouring concrete, working concrete and setting and removing forms from the foundation.
