29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Machine Shop, on or about 10/18/16: Bridgeport milling machine was not equipped with a chuck and serrated tool guard. b) Machine Shop, on or about 10/18/16: Kuraki Boring mill was not equipped with a chuck and tool guard. c) Machine Shop, on or about 10/18/16: Bridgeport end m ill was not equipped with a chuck and tool guard. d) Machine Shop, on or about 10/18/16: Acer milling machine was not equipped with a chuck and tool guard. e) Machine Shop, on or about 10/18/16: Fosdick drill press was not equipped with a chuck and tool guard. f) Weld Shop, on or about 10/18/16: Comet-XT milling machine was not equipped with a chuck and tool guard. g) Weld Shop, on or about 10/18/16: Cincinnati horizontal manual mill was not equipped with a chuck and tool guard. h) Raw Material Stock Room, on or about 10/18/16: Peerless horizontal bandsaw was not provided with an adjustable guard to cover the unused portion of the blade.
