29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): a) At 355 Paul Avenue in St. Louis, MO 63135 (Fabrication Area) - On or about 10/03/2016, employees were exposed to struck-against or crushed-by hazards where powered industrial trucks, such as, but not limited to, a "Towmotor" T50B (Model No. T50B; Serial No. 12N1006) was found defective and in need of repair, as it was missing the factory installed seat belt, and had not been taken out of service until restored to safe operating conditions. b) At 355 Paul Avenue in St. Louis, MO 63135 (Fabrication Areas) - On or about 12/06/2016, employees were exposed to struck-against or crushed-by hazards where powered industrial trucks, such as, but not limited to, a "Toyota" (Forklift 5) was found defective and in need of repair, as it was missing the slip resistant cover on the brake petal, and had not been taken out of service until restored to safe operating conditions.
