29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: a) This violation occurred on or about September 21, 2016, and at times prior thereto; in the production department, line 4; where employees were using 220-Volt, 30-A disconnect without electrical ground to power machines and equipment, exposing employees to electrocution, electrical shock and burns hazards.
