29 CFR 1926.451(f)(1): Scaffold and/or scaffold components were loaded in excess of their maximum intended loads or rated capacities, whichever was less: a) On or about 09/19/16 at 375 Melville Street, Rochester, NY; the scaffold system consisting of a Werner brand ladder jack scaffold, Model WNS-2424, with a rated load of 500 pounds and two support ladders ( a Louisville and a Werner brand 24 foot each with a 300 pound capacity ) was overloaded when three workers, cumulatively weighing approximately 695 pounds were directed onto the scaffold at the same time resulting in a collapse of one of the support ladders with all employees falling approximately 15 feet.
