29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments: A) Terex Utilities, Inc. DBA Terex Services, conducting work at 1300 Marion-Agosta Road, Marion, OH 43302: On or about September 16, 2016, the employer did not provide adequate training to those employees on the subject site on the use of personal protective equipment for the hands, head and body to protect against arc flash hazards when the employee is verifying said de-energization of the Whiting 30-Ton overhead crane and exposed to over 480 volts electrical hazards.
