Presque Isle Wine Cellars, Inc., 9440 West Main Street, North East, PA 16428, on or about September 15, 2016 and times prior thereto: Employees perform service and maintenance activities on equipment including, but not limited to, an auger, crusher and must pump. Employees were not locking out machinery when performing service and maintenance activities.
