29 CFR 1910.27(d)(3): The side rails of through or side-step ladder extensions shall extend 3 1/2 feet above parapets and landings: a) Packaging Room, ladder for accessing the upper deck for the Rovema #2: Side rails for the fixed ladders for Rovema #2 did not extend above the approximate 12' high upper deck landing, on or about 09/10/2016; b) Packaging Room, ladder for accessing the upper deck for the Rovema #1: Side rails for the fixed ladders for Rovema #1 did not extend above the approximate 9' high upper deck landing, on or about 09/10/2016. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
