29 CFR 1910.119(j)(2): Written procedures. The employer did not establish and implement written procedures to maintain the on-going integrity of process equipment. The employer failed to established and implemented written procedures to maintain the on-going mechanical integrity of process equipment, such as but not limited to: a) The employer failed to ensure detailed written procedures were established and implemented for inspection and testing for ammonia detectors in machine rooms in compliance to ASHRAE 15 and IIAR 2. To identified ammonia detectors set points to alarm at 1000ppm and the appropriate TLV-TWA and second detector to alarm and actuate emergency ventilation at 1000ppm. This condition exposed employees to fire or inhalation hazards ammonia vapors.
