29 CFR 1926.1404(h)(9): The A/D director supervising the assembly/disassembly operation did not address the hazards associated with the operation, which include: The potential for unintended movement from inadequately supported counterweights and from hoisting counterweights. During assembly and disassembly of a DEMAG AC 500-1 Telescoping Boom Crane (serial number 84090), employees were exposed to struck-by and crushed-by hazards while standing on and near the counterweight tray while counterweights were being hoisted and lowered.
