29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings shall be effectively closed. a) Tank Car Cleaning Area outside adjacent to the tank cars set on the rails - On or about 7/21/2016, the employer failed to close multiple unused openings in a 480-volt switch cabinet, exposing employees to an electrical shock hazard as the; and, b) Inside the maintenance room in the building across from the Tank Car Maintenance & Repair Shop - On or about 7/21/2016, the employer failed to close multiple unused openings in a 110-volt switch cabinet exposing employees to an electrical shock hazard.
