29 CFR 1910.1030(d)(3)(ii): The employer did not ensure that the employee used appropriate personal protective equipment when there was occupational exposure: a) Throughout facility: The employer did not ensure that employees used appropriate personal protective equipment, including safety glasses with a face mask or a face shield, when there was occupational exposure to blood or other potentially infectious materials when providing dental care, such as, but not limited to, drilling teeth, tooth extraction and scaling of teeth on or about 7/18/16. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
