Executive Order 12196 Section 1-201(a) of the Occupational Safety and Health Programs for Federal Employees: The Head of the Agency did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious harm to employees in that employees were exposed to the hazard of moving machinery where the lighting was not adequate: a) Adequate lighting was not provided for the couplers and clutch/brake units C- 6, C-7, C-8 and C-9. The rotating shafts and couplers could not be seen. b) The florescent lights that are 4 feet above the floor area along the low walkway by the take up rolls in the B-12 & B-15 conveyor belt areas need a cover around the tube to protect against the event of a breakage.
