29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a.) Terminal Ready Mix, Inc. located at 524 Colorado Ave. Lorain, OH; On May 13, 2016, the employer did not ensure the guard on the Dust Collector motor was not easily removed. An employee removed the cover to find grease fittings that had fallen when his hand was caught between a belt and two pulleys, resulting in an amputation of the tip of the right index finger.
