29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: (a)On or about July 7, 2016, the employer failed to ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. Employees working throughout the facility are exposed to hazards including but not limited to; hazards to the face and eyes from spraying and handling chemicals including, but not limited to Elgen Water Based Liner Adhesive, Capital Hardware Elgen 100% Silicone Sealant and Adhesive, Elgen Duct Sealer, 10555 Spiral Lube QD, and OEM/Mechanical - CT10101-5 insulation material, and flying object hazards from cutting, grinding, and fastening metal parts. Employees performing work in the spray area, punch press area, fittings department, and elbow department were observed without eye or face protection. b) On or about September 7, 2016, the employer failed to ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. Employees working throughout the facility are exposed to hazards including but not limited to; hazards to the face and eyes from spraying and handling chemicals including, but not limited to 0555 Spiral Lube QD and flying particle hazards. Employees performing work in the elbow department were observed without eye or face protection. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
