29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) New residential construction on Lot #3, Porch roof: On or about May 31, 2016, and at times prior, the employer exposed employees to fall hazards of 20-feet, 7-inches to the lower level, in that the ladder used to access the lower roof was not extended 3-feet or secured to the structure to allow for safe access on and off the ladder.
