29 CFR 1910.27(b)(1)(ii): The distance between rungs exceeded 12 inches and were not uniform throughout the ladder. At facility, employees accessed upper working platforms using fixed ladders that exposed the employees to fall hazards: a) at building two (2) on the cement/mixer silo, the fixed ladders had a rung distance of 16 inches between rungs; and b) at building five (5) on the concrete forms, the fixed ladders were not uniform throughout.
