29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: Maintenance shop, northeast wall: On or about February 17, 2016, employees were exposed to the hazards of being struck by flying projectiles, while operating a Jet 8 inch bench grinder (model: JBG-8A, serial: 14031931) that that did not have the tongue guard adjusted to protect the employee from the rotating wheel and the grinder opening.
