Mattocks Five, Inc. dba Albion Mill, 73 Canal Street, Albion, PA 16401, on or about February 10, 2016 and times prior thereto: Employees operate a Rollo-Mixer brand batch mixer to make product including, but not limited to, animal feed. The sides and rear of the rotating drum of the batch mixer was not guarded.
