29 CFR 1910.272(g)(1)(i): The employer did not issue a permit for entering bins, silos or tanks in the grain handling facility before the employees entered the bins, silos, or tanks: a.) Mattocks Five, Inc. dba Albion Mill, 73 Canal St., Albion, Pa 16401, Second floor of the Mill Area, on or about February 10th, 2016 and times prior thereto: Employees periodically entered into bins including, but not limited to, cracked corn, shelled corn, and soybean meal. Permits were not issued upon entry into bins.
