29 CFR 1926.859(g): During demolition, continuing inspections by a competent person shall be made as the work progresses to detect hazards resulting from weakened or deteriorated floors, or walls, or loosened material. No employee shall be permitted to work where such hazards exist until they are corrected by shoring, bracing, or other effective means. a) On or about December 2, 2015 one employee was fatally injured and a second employee was hospitalized when the towers supporting a cable coal bridge prematurely collapsed during their demolition. A competent person was not inspecting during the pre-burning operation of the park side tower's North leg exposing employees to struck-by and crushed-by hazards.
