29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: The employer is failing to ensure that the employees are utilizing energy isolation devices in the NZ FF&S area. This failure to utilize energy isolation devices was most recently documented at 1329 West Highway 6, Holdrege Nebraska when an employee was replacing the bar jam cylinder on the 3/10 ML Packaging machine.
