29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about 10/13/2015 in the trailer repair shop and on-site at 6575 Marshall Blvd in Lithonia, GA: Employees working atop trailers that were positioned inside of or contiguous to a building or other structure at heights up to 12 feet 10 1/2 inches from ground level were not provided fall protection.
