29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: a) Throughout facility: The employer did not provide and ensure the use of devices that would isolate, secure or block the energy sources on machines to employees that cleaned, serviced or maintained equipment, including, but not limited to, the doughnut cutters, Belshaw Thermomatic proofer boxes model TM600#23932 serial 1404045 and model TM600 # 22584 serial 14030178, Belshaw Century Gas fryers model C10-24GLH#23929 serial 14030180, Hobart Mixers and Berto Yeast Mixers model 160-AR machine number 171330 and model 160-AR machine number 171329, on or about 10/5/15. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
