29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Location - Johnson Street, N. Andover, MA: On or about 7/7/15, employees were not protected from falling while using a ladder that did not extend at least three feet above the upper landing surface. This violation was most recently observed on 7/7/2015 at a jobsite located 2-4 Johnson Street, North Andover, MA 01845 where roofing operation was taking place, the employees were exposed to falls of over 18 feet where portable ladders side rails did not extend at least 3 feet above the upper landing surface being accessed. To abate this violation, the employer must ensure that all portable ladder systems extend at least 3 feet above the upper landing surface being accessed to protect employees working 6 feet or more above a lower level from falling.
