29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed: On or about June 26, 2015, the employer failed to ensure that guarding was present on the chain driven gears and sprockets located at sections of its Envelope Lines, including the following sections: feed roller, printing, window, side gluing, seal gumming, dryer, top fold, final fold and delivery.
