29 CFR 1910.217(b)(7)(iv): The inch operating means on mechanical power press(es), using part revolution clutches, was not designed to prevent exposure of the worker's hands within the point of operation: On or about June 5, 2015, the employer failed to ensure that Press #111 when operated in inch mode prevented the operator from exposure to the point of operation. The press was operated in inch mode using a foot pedal. The operator's hands were free to reach the point of operation.
